Prevention of hospital admission

At Angel Carers we design admission prevention schemes around all conditions for our clients. Our detailed care plans enable close monitoring of a client’s health. This is coupled with close liaison with local community services to enable us to respond quickly should the client need the help of their Occupational Therapist, GP, Physiotherapist, District Nurse or Community Psychiatric Nurse.

We continuously risk-assess our clients and have various screening tools to help reduce the risk of dehydration, falls, and pressure sores to name a few...
